ABSTRACT

ABSTRACT Objective: To identify the relationship between hemineglect and pain. Methods: We conducted a systematic review with meta-analysis, examining the relationship between hemineglect and pain. The PRISMA criteria and the Covidence platform were used to select articles and extract data. Methodological quality was evaluated, a comparison was made using pain as an outcome measure, assessed with the visual analog scale (VAS). Results: Out of a total of 340 studies, 4 were selected, 2 of which made it possible to unify pain measured through VAS; the effects were summarized using standardized mean differences (SMD), yielding -0.65, with values of [-1.02, -0.29], with statistical significance. Conclusions: There is moderate evidence of the relationship between negligence and pain. Despite the fact that from the clinical care processes it is presented regularly, and even the little scientific information available, it was established that there are specific pathologies that carry negligence and pain.


RESUMEN Objetivo: Identificar la relación entre heminegligencia y dolor. Métodos: Realizamos una revisión sistemática con metanálisis que examina la relación entre heminegligencia y dolor. Se utilizaron los criterios PRISMA y la plataforma Covidence para seleccionar artículos y extraer datos. Se evaluó la calidad metodológica, se realizó una comparación utilizando el dolor como medida de resultado, evaluado con la escala visual analógica (EVA). Resultados: De un total de 340 estudios, se seleccionaron 4, de los cuales 2 permitieron unificar el dolor medido mediante EVA; los efectos se resumieron utilizando diferencias de medias estandarizadas (DME), obteniendo -0,65 con valores de [-1,02, -0,29] con significancia estadística. Conclusiones: Existe evidencia moderada de la relación entre negligencia y dolor. A pesar de que a partir de los procesos de atención clínica se presenta con regularidad y aún con la poca información científica disponible, se estableció que existen patologías específicas que conllevan negligencia y dolor.

ABSTRACT Objective: To verify whether differences exist between groups of Brazilian adults aged 40-59 and 60-75 in respective performance on the Bells Test, given the dearth of literature investigating the relationship between focused visual attention and the age factor. Methods: Eighty-four neurologically healthy adults (half aged 40-59 and half 60-75) with high educational level (40-59 years group: M=17.75 years' education; SD=4.00; 60-75 years group: M=15.85 years education; SD=3.19) were assessed using the Bells Test. Data on accuracy and processing speed were compared between groups by ANCOVA, controlled for the covariates education and frequency of reading and writing habits. Results: There were no significant differences between the age groups. Conclusion: It is suggested that aging influences sustained and focused attention and speed processing after 75 years of age on visual cancellation paradigms, when executive and attentional changes tend to be more marked. Further studies should investigate healthy older and oldest-old adults, as well as groups with low and intermediate educational backgrounds. In addition, Brazilian clinical populations should also be characterized, particularly those with neurological disorders that might have visual hemineglect.


RESUMO Objetivo: Verificar se existem diferenças entre grupos de adultos brasileiros de 40 a 59 e 60 a 75 anos de idade na performance no Teste dos Sinos, uma vez que há uma literatura reduzida que aborda as relações entre a atenção visual focalizada e o fator de idade. Métodos: Foram avaliados 80 adultos (metade deles com idade entre 40 e 59 e outra metade com idade entre 60 e 75) com alto nível educacional (40-59 anos de idade: M=17,75, DP=4,00; 60-75 anos de idade: M=15,85, DP=3,19) e saudáveis neurologicamente, por meio do Teste dos Sinos. Os dados de precisão e velocidade de processamento foram comparados entre os grupos por uma análise ANCOVA, controlando as covariáveis educação e frequência de hábitos de leitura e escrita. Resultados: Não houve diferenças significativas entre os grupos etários. Conclusão: Sugere-se que a influência do envelhecimento na atenção sustentada e focalizada e velocidade de processamento após os 75 anos de idade em paradigmas de cancelamento visual, quando as mudanças executivas e atencionais tendem a ser mais notáveis. Estudos futuros devem investigar adultos saudáveis mais velhos e longevos, bem como, adultos com níveis educacionais baixos e intermediários. Ademais, populações clínicas brasileiras também devem ser caracterizadas, principalmente com transtornos neurológicos que podem apresentar heminegligência visual.

ABSTRACT

O papel de variáveis sociodemográficas na cognição humana, em especial da escolaridade, vem sendo estudado em indivíduos saudáveis. Esta pesquisa objetivou verificar se há diferenças entre grupos de diferentes níveis educacionais no processamento atencional visual avaliado pelo Teste de Cancelamento dos Sinos. Participaram 124 adultos jovens, distribuídos em três grupos, 5-8, 9-11 e 12 ou mais anos de educação formal, emparelhados por idade. As variáveis de acurácia e tempo, assim como os resultados qualitativos de estratégias utilizadas, foram comparados entre grupos (ANCOVA One-Way e Qui-quadrado). Observou-se uma discrepância entre dados quantitativos e qualitativos. Os grupos de diferentes escolaridades não se diferenciaram entre si quanto a acurácia e tempo; houve, porém, diferenças significativas quanto à distribuição de participantes por coluna em que o primeiro sino foi cancelado e pelas estratégias de cancelamento utilizadas. Novos estudos são relevantes com grupos de diferentes idades e com populações clínicas neurológicas e/ou psiquiátricas.


The role of socio-demographic variables on human cognition, particularly education, has been studied in healthy individuals. This research aimed to verify if there are differences among educational groups regarding the visual attentional processing assessed by means of Bells Test. The sample was comprised of 124 young adults, divided into three groups, 5-8, 9-11 and 12 or more years of formal education, matched by age. Accuracy and time data were analyzed by one-way ANCOVA and the qualitative results on strategies were compared between groups by Chi-square. The results showed a discrepancy between the quantitative and qualitative analysis. There were no quantitative differences among educational groups regarding accuracy and time variables. However, significant differences were found in the distribution of participants by chosen column where the first bell was canceled and the used strategies of cancellation. Further studies are relevant including different age groups and clinical samples with neurological and/or psychiatric disorders.

ABSTRACT

Os testes de cancelamento são classicamente utilizados em avaliações neuropsicológicas de atenção e velocidade psicomotora, com a tarefa tradicional de riscar alvos entre distratores. O Teste de Cancelamento dos Sinos, reconhecido internacionalmente por sua contribuição diagnóstica de heminegligência visual, foi desenvolvido com a tarefa de circular todos os sinos o mais rápido possível. Buscou-se verificar se há diferenças de desempenho entre duas versões de aplicação - sinos círculo e traço - em uma mesma amostra de 88 adultos neurologicamente saudáveis, de 19 a 75 anos, com no mínimo cinco anos de escolaridade. Eles foram avaliados na mesma sessão com as duas versões alternadamente. Os escores de acurácia e tempo foram comparados pelo teste Wilcoxon. O tempo da tarefa de riscar foi menor que o tempo da tarefa de circular. Assim, as duas versões não parecem ser totalmente equivalentes. Sugerem-se estudos para padronizar a versão sinos traço, assim como para caracterizar amostras clínicas, principalmente aquelas com ocorrência de heminegligência como sequela.


Cancellation tests are traditionally used in neuropsychological assessment of attention and psychomotor speed, with the traditional task of marking targets. The Bells Test, internationally acknowledged for its contribution to the diagnosis of visual hemineglect, was developed with the task of circling all the bells as quick as possible. This paper aimed at verifying whether there are differences in performance between two versions of application - bells with circles and bells with straight lines - in the same sample consisting of 88 neurologically healthy adults from 19 to 75 years of age, with at least 5 years of formal education. The sample was evaluated in the same session with the two versions in an alternative manner. Accuracy and time scores were compared by the Wilcoxon test. The performance time of the marking task was lower than in the circular version. Thus, the two versions do not seem to be totally equivalent. Studies are suggested to standardize the bells with straight lines version, as well as to describe clinical samples, especially those with hemineglect as a sequelae.

ABSTRACT

Objective To explore possible mechanisms of the cross-over observed in stroke patients comple-ting a line bisection test. Methods Twenty-five stroke patients with left neglect and twenty normal subjects were studied.In the ordinary line bisection test,there are two papers presenting 20 mm and 100 mm lines respectively.On each paper,three lines with the same length are presented at the left,middle and right of the paper.In the line im-age test,the two papers are the same as in line bisection test,but here subjects are asked to set endpoints of each re-al line. Results Replicating earlier findings,length cross-over was observed in the patients with neglect.In the hline image test,those patients neglected lines on both sides but with a much larger proportion on the left.The patientsunderestimated long lines as in previous studies,but they also underestimated very short lines in contrast to earlier findings. Conclusion Foveal neglect sparing may explain these observations.For very short lines,the left end of the line falls into foveal vision and the left part of the line was not neglected.As a result,the crossover effect of line length occurs.

@# ObjectiveTo examine the occurrence rate of visuospatial hemi-neglect of patients with cerebral injury with behavioural inattention test (BIT) (Japanese version),and assess the validity of the BIT test.Methods22 subjects (16 with right hemispheric damage and 6 with left hemispheric damage) were tested with BIT. ResultsThe rate of visuospatial hemi-neglect in 22 patients with hemispheric damage was 36.4% (8 cases) , and that rate in 16 patients with right hemispheric damage was 31.3% (5 cases), in 6 patients with left hemispheric damage was 50% (3 cases).ConclusionThe occurrence rate of visuospatial hemi-neglect appears to be independent from male or female, age, course of disease and education level of study subjects.

ABSTRACT

OBJECTIVE: To investigate the effect of the prism which deviates the optical axis 10 degree rightward in the stroke patients who have visuospatial neglect and to evaluate how long it will last. METHOD: The subjects were 9 left hemiplegic patients who showed the feature of hemineglect. The prism goggle was applied and the patients were trained the midline point direction exercise. It's effect was evaluated by the line bisection test and the Albert test at the pre-training, immediate, 2 hours and 6 hours after the training respectively. RESULTS: The line bisection test and Albert test were not improved significantly at each evaluation time after the training. CONCLUSION: This result revealed the prism therapy does not have a significant effect on the line bisection task and Albert test in the hemineglect patients consistently. The line bisection result in the line located in the left or right side at the 2 hours after the training was improved but it could not exclude the training effect of the repetition of the test.
